In order to be a parent driver, you need to email or turn in hard copies to the front office (Mrs. Montijo, principal’s secretary – dmontijo@oside.us) of ALL three of the following:
1. Personal Vehicle Use Form

2. A copy of the insurance policy (NOT insurance card or proof of insurance, but the info that shows liability amounts)
3. Driver’s License (photos of it if emailed)
